We've been trying to get tax forms for 4 months now, and have gone thru two escalation reviews, with no resolution. The customer service representative were not helpful, with the exception of one person, Zach Olsen.

The others were rude and inconsiderate. We still have not had any luck getting this resolved.

I had the same problem. After my second request I downloaded a blank 1095A from irs.gov and had the marketplace agent read the information to me.

You need the marketplace-assigned policy number if you're going to file a tax return online. get the column totals for lines 21-33.

They told me they mailed my 1095A twice and I never received it. I would give up if I were you.
